I am trying to understand the mechanix of the device, I would be worried the amount of slugging with the sledge would throw off the wrench thread

The hammer component of the head is welded to the two outer bars of the neck. The outer bars are connected to each other with a ring, through which the handle fits. The top of the wrench is welded to the middle bar, the terminal end of which is threaded and fits inside the handle. The rotation of the handle extends the wrench.

I suppose it would also help if I had given some indication as to the scale. Molotok is around two feet long, so the threading is quite large.
