Is the UN just some cover for the Canadian to occupy and annex the areas least likely to have been nuked? And did the show give any reasons why New York, New Orleans, and Omaha weren't incinerated by at least a dozen ICBMs aimed at each?
In the show, the cities aren't hit by ICBMs, but rather jury-rigged nuclear warheads hidden on site. The reason New York is not nuked is explained in the show - it was supposed to be.

Can you imagine how New York would far, even if not hit by a nuke? The resulting panic, millions trying to flee (including a large % of the police force) would result in almost immediate and widspread anarchy.
Add the EMP, fridges and water systems stops, so there would be starvation and water shortage within days, surrounding areas are overrune by refugees and quickly runs out of food.

Does it give anything about the fallout from the rest of BosWash effecting them or why they nuked a small time test ground in Utah? And did the show actually have the United States degenerate to the point that the United Nations came along? Or was it their mystical powers that protected New York?

In the show one of the main characters says that New York was spared because of all the security measures that were put in place in New York after the September 11th attacks.
Very little. And yeah, the US degenerated to the point where the UN intervenes. The thing that spared New York was a miscalculation on part of the conspirators.

Chemical and radiological retaliation at the very least, not accounting that Iran was a nuclear power in 2006 and still is today or that North Korea might decide to hit Tel-Aviv rather than an already hit west coast. Sucks to be them to say the least.
