Yes the 3dsmax plug-in Phoenix by Digimation was used to make the flame.
It was my first time using it and just played with the setting until achieving something I liked.

To answer your questions about InfinityKnot --

Yes, the material you guessed is correct. As for the "Knot" I morphed a single Tube shape, then copied it 3 times, I think... then pasted and rotated them until they looked right.
It took a bit of time, but I liked the outcome. Glad you liked it!

That is certainly one of my favorite pictures so far. I do that exact thing, disecting other people's work to see how much effort was put into it.
This picture took about 3 days I think. I made my own material for it, and rendering took quite awhile.
