I really wish it had more of a background and a light source, but the draping on the skirt came out nice

It depends on the mood you want to set and the situation going on the the pic. Here it's just a figure, without much to embellish it so it's a bit hard to say...
If you want to keep the white/sterilization thing, you could draw in a hospitable operating room- or maybe go another way with it and throw in a grave sight, or some sort of sacrificial backdrop. Or prehapse an elaborate window of some sort.
Make up a little back-story for yourself and see what fits best- I can't really say what it should be for you without knowing more about the pic/what it is you're trying to convey.

In all honesty I don't much either but if you're really stuck it's a good thing to consider.
