I don't think that has been answered definitively, though I think Freud, who defined the phenomena in the first place, still described it best, and came as close as anyone to detailing the processes surrounding it. Of course the term has become diluted and bastardized now to mean any sort of sexual kink or depravity, such as De Sade catalogues in 120 Days of Sodom. But I usually use it in closer to the original Freudian sense.

I can tell you this. At a very early age, as early as I can remember, I fantasized about undressing my female schoolmates or neighbourhood friends. Their shoes, naturally, would be the starting point for the fantasy. Also I have always found the smell of leather pleasant even evocative. So I liked the smell of girls' shoes along, in general, with the smell of girls. My earliest memory of the sight of a girl's shoe sending me into a sexual swoon was in third grade.
