Could you explain what Hex Journal is? I couldn't find it in Google and I've never heard of it before. 
Part of my idea is that the spiritual part of a human is composed of three separate parts. The part that is getting reincarnated is called the soul and it is the part of the spirit that allows it to control the physical body. It has memories, but it isn't the structure that forms them, only the part that relays memories to the body. So if the soul has its memories wiped clean, the body won't remember anything but the Core(the part that is the definite self) of the spirit will. So when the soul part of the whole spirit is taken, it will have the memories of the last individual, but the actually Core of the reincarnated individual is a whole new person. If you want me to explain it more, I would like you to ask me. I'm trying to work on other projects right now and I don't want to have to juggle more than I already have.
