Yes, looks like you're on the latest version. Can you try these:
1. Go to the Settings tab
2. Set "Config mode" to Advanced
3. Scroll down and click Reset -> Factory Reset.

ive done factory re set and re installed the one click and restarted my fire fox, it seems to still not be working and even tho it lists as enabled, the scrip is showing as not running

It'll say OCLB is running when you're on a DA page. Can you check if Greasemonkey has anything installed? It must have an older version of OCLB installed.
I was having the same problem but this fixed it. Thank you again for such an awesome extension I can't live without. 
uninstalling everything from greasemonkey fixed it, thank you!!
